A debrief should include:

    • A conversation among the participating team members as soon as practical after the activity, so recollection is as accurate as possible. 
    • A listing of what went well, especially what the team did well to adapt to a less than desirable situation. This should be captured in the SMS and shared with the organization, so everyone can learn good techniques to improve overall resiliency. 
    • What did not go so well, especially, what errors where made and why. Take your time and determine contributing factors such as fatigue, distraction, pressure, stress, complacency, less than desirable fitness for duty, lack of resources, lack of effective communication, lack of effective teamwork, ineffective training, etc. These should be captured in the SMS so the organization can systematically analyze, assess, and make needed corrections.
    • What can be done in the future to improve the operation? Make recommendations for improvement, since front line personnel often have the best solutions for continual improvement and addressing issues that can become threats. 

NOTE: When personnel are trained properly in debriefing, the process only takes 5 to 10 minutes, unless unusual circumstances require a more in-depth discussion.
