While a large percentage of aviation accidents are the result of the all-too-frequent Human Error, you needn’t dig too much deeper to find Organizational influences that played a huge role in allowing the accident to occur. In this week’s session, we will review an actual accident event and reveal how easy it is to lose sight of the power the organization plays leading up to an accident.
